What to Submit:

Active duty members of the military are eligible for a reduced fee of $250. Please apply through the Military Traditional Track button and submit a copy of your military ID to verify your active duty status.

Canadian and International Applicants who do not practice in the US may review provisions for prerequisites and continuing education obtained outside the United States.

Withdrawals can be made at any time prior to the exam; no refund will be made after the stated date.
 

Continuing Education Requirements

A total of 50 CE credit hours must be submitted at the time of application. The ABDSM will accept 35 approved CE hours at the time of application; however, if you choose to submit an application with fewer than 50 CE credit hours, the remaining CE must be submitted to the ABDSM by the stated date.
 
CE credits must meet all of the following criteria:
  • Science-based education in dental sleep medicine or sleep medicine
  • ADA CERP recognized or AGD PACE approved
  • Course provider is a non-profit organization or accredited university
  • Applicant is a participant in the course and not the instructor
  • Earned within the three (3) years prior to the application deadline
 
Required documentation must be submitted for each course:
  • CE verification letter or certificate issued by the course provider (CE transcripts are not accepted) 
                    AND
  • Educational objectives or a detailed agenda that shows the course was science-based education in dental sleep medicine or sleep medicine (CE verification letters or certificates not accompanied by course objectives or agenda are not accepted)

Applicants may submit up to 10 credits that are AMA PRA Category 1 Credits TM in sleep medicine in place of ADA CERP or AGD PACE credits. Applicants may also submit up to 10 credits from self-instructional or electronically mediated activities.Credit hours in practice management and billing are not accepted. Credit hours in temporomandibular disorder are accepted only when the course objectives clearly show the course had a prominent dental sleep medicine focus.


Documented Time in a Sleep Center

10 hours of observation are required and the hours must have occurred within the three (3) years prior to the application deadline. You must submit the Observation Hours template, which must be signed by a physician who is board-certified in sleep.
 
The applicant must experience direct observation of a sleep medicine clinic or lab with sleep medicine medical staff that includes at least 3 of the following:
  • PSG preparation/set-up with a patient
  • Data acquisition of a PSG
  • Scoring and interpretation of PST
  • Clinical interaction of a physician or nurse practitioner with a patient
  • Therapeutic intervention with CPAP
  • Assessment of results of therapeutic intervention
It is the applicant’s responsibility to confirm that the physician that signs off on the hours is board-certified in sleep.
 

Letters of Recommendation

 Applicants must submit 2 letters of recommendation signed and dated within two (2) years of the application deadline, using the provided template. The physicians who sign the letters must be board-certified in sleep medicine and must be someone you have worked with. The letters must describe the nature of your working relationship and must include a clear recommendation for the applicant to sit for the ABDSM examination.
 

Business Associate Agreement

In compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), all Traditional Track applicants must sign and submit a copy of the Business Associate Agreement.

 

Case Studies

Each applicant must submit a total of ten (10) documented oral appliance cases, distributed as follows: five (5) detailed cases and five (5) spreadsheet cases. All case study critera can be found in the current exam guidelines. Applicants are responsible for addressing all criteria in their case submission.

Canadian and International Application Provisions
Applicants who do not practice in the United States may submit CE credits that are not ADA CERP recognized or AGD PACE approved; however, the CE credits do need to meet all other ABDSM requirements.  The ABDSM will accept letters of recommendation and sleep observation documentation that are signed by either a physician who is board-certified in sleep medicine OR experienced in sleep medicine. If an applicant submits letters of recommendation or sleep observation documentation signed by a physician who is not board-certified in sleep medicine, the physician’s credentials must be documented in order to confirm the experience in sleep medicine.








 *Online application
The online application will ask you for your contact information and fields in which you will upload copies of the required documentation, so you will want to make sure you have first saved all the documents to your computer. The application also includes terms and conditions that you should review before attesting to your agreement with them.
 
**Dental License with Expiration Date
Provide a copy of your dental license that shows the date of expiration. If the dental license you submit expires prior to May 1, 2020, you must submit a copy of your renewed license to the ABDSM. Applicants will not be allowed to sit for the exam without proof of a valid dental license on file with the ABDSM.


Continuing Education Verification
It is the applicant’s responsibility to verify the non-profit status of a course provider before submitting the credits. To confirm a CE provider’s status, visit GuideStar’s website at www.guidestar.org. All IRS-registered non-profit organizations are listed on GuideStar.
